Dear Sir/Madam

I make a Subject Access Request under The Data Protection Act to the Computer Data Service Provider for Backups, and Walsall Council for all information regarding Tia O’Gorman, Taylor O’Gorman and Philip O’Gorman, and myself, Philip Thompson.

I request the Computer Data Service Provider for Backups and Walsall Council provide:

1.   All data from 2004 to present, including all social work records and SWIFT records regarding myself, Philip Thompson, including emails, reports, on-line framework logs, call logs, telephone calls, recorded conversations etc.

2.   All data from 2004 to present, all social work records and SWIFT records regarding Tia O’Gorman, Taylor O’Gorman and Philip O’Gorman, including emails, reports, on-line framework logs, call logs, telephone calls, recorded conversations etc.

Please send, by both email and by post, all data in its original formats, .doc, .msg (for emails), etc.

I enclose the statutory fee of £10. You have 40 workings days to respond.
